Creating patients

In this section, you will learn how to create a new patient. You will perform this task before taking the first set of baseline photos. In other words, this step must be completed only once per patient.

Steps​

To create a patient, you will need to follow these steps.

Open the Patients section​

Once you are logged in, click the Patients section on the left sidebar. In the following image, you can see the Patients section highlighted.

Click Add​

Once you are in the Patients section, you will see a list of all the participants in the study for your site. On the top right, click the green button labeled Add.

This list will be empty at first

The first time you open this view, the list of participants will be empty because you have not created any participants yet. Please note that only the participants you have created will appear here.

Click the

Add

button. This will open a form to create a new participant.

Fill in the form​

Once you click the

Add

button, you will find a form. Here, you will need to fill in the following fields:

  • Site code: CV5-CC#####
    • Description: CV5 followed by the two-digit country code followed by five digits.
    • Examples: CV5-ES10001, CV5-AR10001, CV5-BE10007
  • Participant code: CC#########
    • Description: Two-digit country code followed by nine digits.
    • Examples: ES100016001, AR100016001, BE100076003

"#" refers to a number from zero to nine.

Fill in this form as shown below:

Basic information

To add the participant code and the site code, simply type in the values.

Ignore Primary Doctor and Referral sections

Please disregard the Primary doctor and Referral sections. They are not relevant to this study.

Save the participant​

Now that you have added all the information about the participant, click the

Add

button to save the participant. This button was previously disabled but is now enabled because you have filled in all the required fields.
